L1061 - DECLARATION RE: DEFAULT IN PAYMENTS/PARTIAL SATISFACTION. This form is used in unlimited civil cases in the Orange County Superior Court when a judgment debtor fails to make court-ordered installment payments. This form allows the judgment creditor or the creditor’s attorney to declare that a judgment was entered, outline the installment payment terms, state the amount received to date, and identify the missed payment. Based on the default, the creditor may request issuance of a writ of execution. The form also includes sections to document partial satisfaction of judgment, including information about recorded Abstracts of Judgment or Notices of Judgment Lien.
