Guardianship Investigation Questionnaire {FL-05}

FL-05 - GUARDIANSHIP INVESTIGATION QUESTIONNAIRE. This is a Sutter County Superior Court form completed in connection with a guardianship petition and submitted to the Court Investigator for review. The comprehensive questionnaire collects detailed information about the proposed guardian, the child or children involved, household members, employment history, childcare arrangements, medical and mental health providers, criminal history, Child Protective Services involvement, and family relationships. It also includes an authorization allowing the Court Investigator to obtain relevant records and information during the guardianship investigation. The form is intended solely for investigative purposes and is not shared with outside parties.
