This is a San Mateo County Superior Court Family Court Services form used when a support person attends mediation at the request of a party who alleges they are a victim of domestic violence. The form requires the support person to acknowledge their role, confirm they are at least 18 years old, and certify that they are not the attorney of record for either party.
