This is a Sacramento County Superior Court form used by a party who has already participated in mediation and seeks exemption from the court’s Mandatory Settlement Conference Program under Local Rule 2.80. The form identifies the requesting party, the date and type of mediation completed, and the mediator involved. After reviewing the request, the court may vacate a scheduled Mandatory Settlement Conference, order that no settlement conference be scheduled in the case, deny the request, or issue other appropriate orders.
