This is a Sacramento County Superior Court form used by court-appointed arbitrators to certify their eligibility to serve in a civil arbitration case. The form requires the arbitrator to disclose any grounds for disqualification under California Code of Civil Procedure Section 170 et seq. and confirms that neither the arbitrator nor any disqualified member of their law firm has a conflict that would prevent service. It also includes a certification that the arbitrator has not solicited, and will not solicit, private mediation business from any party involved in the case.
