This is a Sacramento County Superior Court Civil Division form used by a party who wishes to reject an arbitration award and request a new trial before the court. The form allows plaintiffs, defendants, or other parties to notify the court that they do not accept the arbitrator’s decision and seek a trial de novo pursuant to California Code of Civil Procedure section 1141.20 and California Rules of Court, rule 3.826. It includes space to identify the parties making the request and contains a Proof of Service section to document service of the filing on all required parties.
