MM12 (E) – APPOINTMENT OF A REPRESENTATIVE. This is a World Intellectual Property Organization (WIPO) Madrid System form used by applicants and holders of international trademark applications or registrations to appoint a representative to act on their behalf before WIPO. The form allows the applicant or holder to designate an authorized representative and provide the representative’s contact information, including name, address, email address, and optional telephone number. It may be used for one or multiple international applications or registrations owned by the same applicant or holder.
