DS-5525 - STATEMENT OF EXIGENT/SPECIAL FAMILY CIRCUMSTANCES FOR ISSUANCE OF A U.S. PASSPORT TO A CHILD UNDER AGE 16. This is a U.S. Department of State form used when a parent or legal guardian cannot obtain the required notarized consent of the non-applying parent or guardian for a child’s passport application. The form allows the applying parent or guardian to explain exigent circumstances, such as a time-sensitive emergency affecting the child’s welfare, or special family circumstances that make obtaining consent exceptionally difficult or impossible. Applicants must provide information about the child, the non-applying parent or guardian, prior court orders, incarceration status, efforts made to contact the non-applying parent, and a detailed explanation of why consent cannot be obtained.
