FORM K - GUARDIAN ADVOCATE EDUCATION REQUIREMENTS. This is a Florida probate court form used by an appointed Guardian Advocate to notify the court that the required education and training requirements have been completed. Pursuant to Florida Statutes sections 744.3135 and 744.3145, the form allows the Guardian Advocate to certify that they completed the mandatory training within four months of their appointment. The form identifies the guardian advocate, the ward, the appointment date, and the training provider, and requires the filing of a certificate of completion as supporting documentation.
