JC-109 - REQUEST AND ORDER TO APPOINT SONOMA CASA (COURT APPOINTED SPECIAL ADVOCATES). This form used in the Superior Court of California, County of Sonoma, to request the appointment of a Court Appointed Special Advocate (CASA) in a juvenile case. This form may be submitted by the child, a CASA program representative, or another authorized individual who believes that CASA involvement would serve the child’s best interests. It includes a declaration under penalty of perjury supporting the request.
