This is a Pennsylvania civil form used in the Court of Common Pleas of Lehigh County to request entry of a judgment of non pros (non prosequitur) against a plaintiff who has failed to file a complaint or proceed within the time required by law. The filer certifies compliance with Pa.R.Civ.P. 237.1, confirming that written notice of intent to file was sent at least 10 days prior to filing to both the plaintiff and their attorney, if any. The filer must also verify that no complaint was filed within 20 days following service of the Rule.
