Description
BMV 5791 - FIFTY HOUR AFFIDAVIT. This form issued by the Ohio Department of Public Safety, Bureau of Motor Vehicles, is used to verify that a temporary permit holder has completed the minimum state-required supervised driving experience necessary to apply for a driver’s license. This includes at least 50 total hours of supervised driving, of which 10 hours must occur at night (defined as one-half hour after sunset to one-half hour before sunrise). The affidavit must be signed by an eligible adult, who may be the parent, guardian, custodian, or an individual over age 21 acting in loco parentis. This adult confirms under oath, before a notary public, that the driving log is truthful and complete. The affidavit also contains a Supervised Driving Log where the supervising driver records dates, times, and durations of each driving session, including whether it occurred during the day or night.
