Form DFS-F3-DWC-24 - DEPARTMENT AND STUDENT AGREEMENT FOR SPONSORSHIP OF TRAINING AND EDUCATION. This Florida Department of Financial Services (DFS) form is used by the Division of Workers’ Compensation to outline responsibilities between the Department and an injured employee (student) when sponsoring training or education programs under workers’ compensation rehabilitation. The agreement specifies that students must attend full-time, maintain at least a 2.0 GPA (or higher program standard), report grades and attendance regularly, and cover personal expenses such as transportation and non-approved supplies. The Department agrees to sponsor only approved programs, pay authorized tuition, fees, and special supplies, and determine eligibility for continuation or readmission after documented hardships.
