DS-11 - APPLICATION FOR A U.S. PASSPORT. This form is used by individuals who are applying for a U.S. passport book and/or passport card for the first time, or who are not eligible to use a renewal form. This includes applicants under age 16, applicants whose previous passports were issued when they were under 16, applicants whose previous passports were issued more than 15 years ago, and those whose passports have been lost, stolen, or damaged. The form must be submitted in person at a passport acceptance facility, passport agency, or U.S. embassy or consulate (if applying abroad). To complete the application, the applicant must provide proof of U.S. citizenship, proof of identity, a recent passport photo, and supporting documents. For minors, proof of parental relationship and parental consent or awareness may also be required. The application collects personal information such as name, date of birth, Social Security number, contact details, travel plans, and emergency contact information. Fees must be paid at the time of application, with additional fees for expedited service or special delivery if requested.
