CN-500 ANCH - CINA THERAPEUTIC COURT – PARTICIPANT ACKNOWLEDGMENT OF PROGRAM REQUIREMENTS. This form is used in the Alaska Superior Court to document a participant’s understanding and agreement to the conditions of the Child in Need of Aid (CINA) Therapeutic Court program. It outlines a series of responsibilities that the participant must initial to confirm they understand and accept, including requirements such as being honest with the court, abstaining from drugs and alcohol, attending all treatment and court-ordered services, appearing in court on time, maintaining confidentiality about other families, and submitting to regular drug and alcohol testing. The participant also agrees to notify the court and relevant parties about any prescribed medications, understands that rewards may be given for progress, and acknowledges that noncompliance may lead to sanctions. The participant consents to sign all necessary confidentiality releases to allow communication between the treatment providers and the CTC Team.
