This form is used in Yolo County by a partner who is leaving a partnership that operates under a fictitious business name. The form requires details of the business name, date of original registration, the principal business address, and the names and addresses of the withdrawing partner(s). Once filed, this statement must be published in a local newspaper in the same manner as the original fictitious business name statement.
