REQUEST FOR REDACTION OF EXEMPT PERSONAL INFORMATION FROM NON-JUDICIAL PUBLIC RECORDS. This form is used by individuals to request that certain personal information be removed or redacted from public records maintained by the Polk County Clerk’s Office in Florida. It allows eligible individuals—including current or former government employees, their spouses or children, victims of violent crimes or domestic violence, and other protected persons under Florida law—to protect sensitive information such as home addresses, telephone numbers, dates of birth, social security numbers, employment locations, school or daycare information for children, and personal assets from public disclosure. The form requires the requester to provide identifying information, specify which documents or records need redaction, and acknowledge that the request itself is a public record while agreeing to indemnify the Clerk’s Office against any claims arising from the request.
