REQUEST FOR REDACTION OF EXEMPT PERSONAL INFORMATION FROM NON-JUDICIAL PUBLIC RECORDS. This form is used by individuals to request that certain personal information be removed or redacted from public records maintained by the Polk County Clerk’s Office in Florida.
