ATJ 2804.4 - ADDITIONAL PROOF OF DELIVERY. This form is used in Illinois Appellate Court proceedings to document the proper service of court documents under Illinois Supreme Court Rule 11. It is an extension of the standard proof of delivery section when a party needs to serve documents to more than the initial recipients listed.
