ATJ 2804.4 - ADDITIONAL PROOF OF DELIVERY. This form is used in Illinois Appellate Court proceedings to document the proper service of court documents under Illinois Supreme Court Rule 11. It is an extension of the standard proof of delivery section when a party needs to serve documents to more than the initial recipients listed. The form requires the sender to specify the name of the document being delivered, the recipient’s name, mailing address, and email address, as well as the method of delivery—such as email, electronic filing service provider (EFSP), mail, third-party carrier, personal delivery, or mailing from a correctional facility. The sender must also indicate the date and time the document will be sent. The form includes a certification under 735 ILCS 5/1-109 that the information provided is true and correct, and warns that knowingly submitting false information constitutes perjury, a Class 3 Felony.
