This form is filed with the Texas Secretary of State and allows an elected or appointed officer to refuse all or specific types of remuneration, such as salary, per diem, expense reimbursements, or fees. The form requires the officer’s identifying information, the office involved, and a clear statement specifying whether all or only certain compensation is declined. It must be signed under oath before a notary public. A declination of remuneration is effective on the date it is filed with the office of the secretary of state. A declination filed before a person has qualified for office may not be revoked during the term of office to which the person is appointed or elected.
