CCCP 0214 - STATEMENT OF RIGHT TO PETITION FOR TERMINATION OF ADJUDICATION OF DISABILITY, REVOCATION OF LETTERS OF GUARDIANSHIP OR MODIFICATION OF DUTIES OF GUARDIAN. This form is a court-issued notice provided to a person who has been adjudicated as having a disability and is currently under guardianship. This form is used in the Probate Division of the Circuit Court of Cook County, Illinois, and serves to inform the individual of their legal rights under Section 11a-20 of the Illinois Probate Act of 1975.
