CC55 - PHYSICIAN’S REPORT. This form is used in the Circuit Court for the Twenty-Third Judicial Circuit in Kendall County, Illinois, during guardianship proceedings involving an alleged disabled person. The form is completed by a physician licensed to practice medicine in Illinois and is based on a medical examination conducted within three months of filing the guardianship petition. The purpose of this report is to provide the court with a professional medical assessment of the respondent’s physical and mental condition, including their ability to make personal and financial decisions, adaptive behavior, social skills, and educational status if applicable. The physician is also asked to give an opinion on whether the respondent is totally or partially incapable of making such decisions and to recommend the most appropriate living arrangement or treatment plan.
