INFORMATION SHEET. This form is used to provide the Surrogate's Court with essential details about the decedent and their estate. It collects information such as the decedent’s full name, aliases, last known address, date of death, marital status, and whether the decedent left a Will or codicil. The form also requires a list of all next of kin, including their names, relationships, addresses, and the ages of any minors. If someone is applying to be appointed as Executor or Administrator of the estate, their contact information must also be included. For estates without a Will, the form asks for a listing of all assets held solely in the decedent’s name (e.g., real estate, bank accounts, vehicles, stocks, or bonds), along with outstanding debts such as funeral expenses, medical bills, or credit card balances.
