The plaintiff alleges that the defendant issued a written check for a specific amount, which was delivered to the plaintiff but was later returned unpaid by the bank. The plaintiff still possesses the check and has not received payment. The form allows the plaintiff to optionally assert a claim under Florida Statute §68.065 by stating that a written demand for payment was sent by certified or registered mail and that 30 days have passed without payment.
