NOTICE TO LEAVE PREMISES. This form is used in Ohio under Ohio Revised Code Section 1923.04 to notify a tenant and all other occupants that they must vacate the rental property within three days of receiving the notice. This 3-day notice is a required first step before a landlord can file an eviction action in court to regain possession of the premises.
