171P-37 - STATEMENT OF RIGHT TO PETITION FOR DISCHARGE OF GUARDIAN OR MODIFICATION OF GUARDIANSHIP ORDER. This form is used by the Circuit Court of the Nineteenth Judicial Circuit in Lake County, Illinois, to formally notify an individual who has been adjudicated as an adult with a disability that they have the legal right to petition the court for changes to their guardianship. After a guardian has been appointed through a court hearing, the person with a disability receives this form which explains that they can request the termination of their legal disability status, the removal of the appointed guardian, or modifications to the guardian’s powers and responsibilities. This right is granted under Section 11a-20 of the Illinois Probate Act. The individual may initiate this request at any time, and it can be done formally or informally, including by sending a letter to the Presiding Judge of the Probate Division. The form also indicates whether the notice has been personally delivered or is to be mailed by the Clerk of the Court.
