This form is used in the Alaska District or Superior Court by a restitution recipient who chooses to opt out of the Alaska Court System’s restitution collection process. By submitting this form the recipient is formally notifying the court that they do not want the court to collect the restitution judgment on their behalf. Instead, they will take responsibility for collecting the owed restitution themselves using the court’s standard post-judgment execution procedures. The individual must provide their signature, contact information, and case number to complete the form.
