DECLARATION OF SERVICE. This form is used in the State of Nevada by a landlord or their authorized agent to certify and document how a legal notice—such as a Notice to Pay Rent or Quit, No Cause Notice, Breach of Contract, Nuisance, or Unlawful Detainer Notice—was properly delivered to a tenant. The form includes the names and contact information of both the landlord and tenant(s), and it requires the person serving the notice to indicate the exact method of service. The three acceptable methods are: personal delivery to the tenant, leaving the notice with a person of suitable age and discretion at the tenant's residence and mailing a copy, or posting the notice in a conspicuous place on the rental property and mailing a copy to the tenant. The server must declare under penalty of perjury that the details provided are true and accurate. If the notice is mailed, specific legal rules apply—such as not counting the day of mailing and adding three additional calendar days for service to be considered complete.
