D33-DOM 005 - EMPLOYER AFFIDAVIT. This form is used in the North Carolina District Court as part of a civil case involving an employee—typically in family law matters such as child support, spousal support, or financial disputes. The form is completed by a payroll officer or other qualified representative of the employer to provide verified employment and income information about a party in the case (either the plaintiff or defendant).
