This Eastern District of Kentucky form is used when a debtor in Chapter 12 bankruptcy seeks a hardship discharge under 11 U.S.C. § 1228(b). This certification affirms that all statutory requirements have been met, including payment of all domestic support obligations due prior to the certification date and confirmation that the provisions of 11 U.S.C. § 522(q)(1) do not apply.
