WAIVER AND DIRECT PAY OF SUPPORT PAYMENTS. This form is used by the 54th Judicial Circuit Family Division in Michigan allows a payee (the person receiving child support) to formally request a credit to the support account for either a waived portion of past-due support or a payment that was received directly from the payer (the person responsible for paying support), bypassing the Friend of the Court (FOC). The payee must choose one of two options: either waiving a specific dollar amount of unpaid support or confirming that they received a direct payment from the payer. This form ensures that FOC records accurately reflect such transactions and that the waived or credited amount will no longer be collected by the FOC.
