This form is used to reinstate and reincorporate legislatively or judicially created not-for-profit corporations in Florida that were automatically dissolved by operation of law on July 2, 1992, under Section 617.1623(1), Florida Statutes. These corporations were established prior to September 1, 1959, and were never included in the records of the Florida Division of Corporations.
