JC-1611 - DISPOSITIONAL ORDER – PROTECTION OR SERVICES (CHAPTER 48). This form is used by the Wisconsin Circuit Court after a child has been found to be in need of protection or services under §48.13 of the Wisconsin Statutes. Following a dispositional hearing, the court uses this form to officially order the terms of the child’s placement and supervision. The form outlines whether the child will remain in the home or be placed out-of-home, and it documents the court’s findings on reasonable efforts made to prevent removal and achieve permanency.
