WC 1 - EMPLOYER’S FIRST REPORT OF INJURY. This form is used by the Colorado Department of Labor and Employment, Division of Workers’ Compensation, to document and report any work-related injury or occupational disease. It must be completed by the employer whenever an employee is injured on the job, regardless of how minor the injury may seem, and is required to be submitted to the employer’s insurance company within the specified time frames.
