REPORT OF MEDIATOR. This form is used in the New York State Unified Court System – Ninth Judicial District to document the outcome of court-referred mediation sessions. Mediators complete this form after mediation to indicate whether the case was fully resolved, partially resolved, unresolved, or deemed inappropriate for mediation. The report also records session format (in-person, video, or telephone), referral and session dates, and reasons if mediation did not proceed. Mediators must not include substantive case details.
