AAN - AMENDED CERTIFICATE OF ASSUMED NAME (DOMESTIC OR FOREIGN BUSINESS ENTITY). This Kentucky Secretary of State form lets domestic or foreign business entities amend an existing, active certificate of assumed name on file—such as updating the entity’s real name, changing partners in a general partnership, or revising the address on the certificate. It cannot be used to change the assumed name itself or to amend an expired or withdrawn certificate; a new certificate of assumed name is required in those cases. The filing fee is $20, payable to the Kentucky State Treasurer.
