DIV810 - FELONY NOTIFICATION AFFIDAVIT. This form is used in the State of Minnesota by individuals who are applying for a legal name change and who have a felony conviction. Pursuant to Minnesota Statutes § 259.13, the applicant must notify the prosecuting authority that obtained the felony conviction of their intent to change their name. If the conviction was issued outside of Minnesota or by a federal court, the applicant must also notify the Minnesota Attorney General. This affidavit serves as proof that the required notice was properly served and that at least 30 days have passed since the notification. The applicant affirms that the request for a name change is made in good faith and not to commit fraud or mislead others. Submitting false information on this form is a gross misdemeanor.
