PTC-200 - SCHEDULE SUPPORTING APPLICATION FOR APPROVAL OF AND ACQUISITION OF CONTROL. This form is used to support an application submitted to the Nevada Gaming Commission and the Nevada Gaming Control Board for the approval of an acquisition of control involving a publicly traded corporation. This form is required under NGC Regulation 16.200 and must include all information specified in NGC Regulation 16.210. It applies in situations where a publicly traded corporation seeks to acquire control of a corporate gaming licensee or affiliated company, or when an individual seeks to acquire control of a publicly traded corporation that is itself an affiliated company. The form requires detailed information about the publicly traded corporation as mandated by NRS 463.635(1)(b), unless such information is already on file and incorporated by reference. Applicants must also describe the full terms and provisions of the transaction and disclose any material changes in the management and operations of both the publicly traded corporation and the licensee expected to result from the change of control.
