PTC-200 - SCHEDULE SUPPORTING APPLICATION FOR APPROVAL OF AND ACQUISITION OF CONTROL. This form is used to support an application submitted to the Nevada Gaming Commission and the Nevada Gaming Control Board for the approval of an acquisition of control involving a publicly traded corporation. This form is required under NGC Regulation 16.200 and must include all information specified in NGC Regulation 16.210.
