CCP 1004 - NOTICE TO INTERESTED PARTIES REGARDING PETITION FOR LETTERS OF ADMINISTRATION (OR LETTERS OF ADMINISTRATION WITH THE WILL ANNEXED). This form is used in the Probate Division of the Circuit Court of Cook County, Illinois, to inform individuals who have a legal interest in the estate of a decedent that a petition has been filed seeking the appointment of an administrator. This may include a petition for general letters of administration (if there is no will) or for letters of administration with the will annexed (if there is a will but no named executor).
