RESIDENTIAL FORECLOSURE MEDIATION QUESTIONNAIRE. This is a form issued by the Circuit Clerk of LaSalle County, Illinois, to assist homeowners involved in foreclosure proceedings. The purpose of the form is to collect financial and situational information from the property owner to determine eligibility for court-sponsored foreclosure mediation. Mediation is a free, confidential, and voluntary process that gives homeowners and lenders the opportunity to discuss possible resolutions to avoid foreclosure, such as loan reinstatement or modification. The questionnaire gathers detailed information about the homeowner’s income, expenses, number of missed mortgage payments, property taxes, insurance costs, other owned properties, and reasons for default, such as job loss or illness. It also asks whether the homeowner wants to keep the property or would consider alternatives like selling the home, consenting to foreclosure, or deeding it to the lender. This form must be completed and brought to the foreclosure mediation prescreening meeting, as noted in the summons.
