NOTICE OF APPOINTMENT OF REGISTERED AGENT (M.G.L. c. 94F, § 3). These forms are used in Massachusetts to appoint a Registered Agent for a Tobacco Product Manufacturer (TPM) under M.G.L. c. 94F, § 3. The "Notice of Appointment of Registered Agent" is completed by the TPM, appointing a Registered Agent in Massachusetts authorized to accept legal documents. It requires notification to the Attorney General if the Registered Agent changes or terminates. The form must be signed by an authorized TPM officer in the presence of a notary public. The "Acceptance of Appointment as Registered Agent" is then filled out by the Registered Agent, confirming acceptance of their role and ability to receive legal documents for the TPM.
