Description

APPLICANT FACT SHEET - ADMINISTRATION AD PROSEQUENDUM. This form is used in the State of New Jersey to initiate the process of obtaining letters of administration ad prosequendum following a person’s death caused by a wrongful act, such as medical malpractice or an automobile accident. This form is submitted to the Surrogate’s Court, usually in the county where the deceased resided or, if they resided out of state, where the fatal incident occurred. The purpose of this type of administration is to allow a legal representative—typically a surviving family member or next of kin—to file a wrongful death lawsuit on behalf of the decedent’s estate. The form gathers essential information including the decedent’s personal details, cause and location of death, potential wrongful party involved, and the names and relationships of all next of kin. It also requires the applicant to provide documentation such as a certified death certificate and government-issued ID, along with the filing fee. The applicant may complete and return the form in person or by mail or email. www.FormsWorkflow.com
