Description
PROBATE SUCCESSOR FACT SHEET. - APPOINTED EXECUTOR UNABLE TO SERVE. This form is used when the executor named in a will is unable to serve and a successor named in the will applies for appointment. It gathers decedent details (name, date of death, street address—no P.O. Box), will/codicil dates, the originally appointed executor(s), and successor-applicant contact information. The form charts family relationships (spouse/partner status; children and any predeceased children’s descendants; parents; siblings; nieces/nephews), lists beneficiaries, and identifies any minors receiving assets. Submit by email, mail, fax, or in person; the Surrogate will review and then contact you to schedule an appointment. A fee is collected at the appointment (cash or check). www.FormsWorkflow.com
