NPM001 - NON-PARTICIPATING TOBACCO PRODUCT MANUFACTURER CERTIFICATE OF COMPLIANCE. Pursuant to R.C. 1346.02 and 1346.05 and Ohio Adm.Code 109:8-1-01 thru 109:8-1-03. This form is used to certify compliance with Ohio's tobacco laws for manufacturers that do not participate in the Master Settlement Agreement. It is required under Ohio Revised Code sections 1346.02 and 1346.05, as well as related administrative code provisions. This form serves several purposes. It identifies the non-participating tobacco product manufacturer, including company details, contact information, and relevant permits. It certifies whether the manufacturer is submitting an annual, supplemental, initial, or final certification related to sales and listing on the Ohio Tobacco Directory. It declares the specific brand families being certified for sale in Ohio, along with evidence of compliance with escrow payment requirements. It also requires the manufacturer to provide escrow account details, including account number, bank information, and proof of deposits made in accordance with Ohio’s tobacco escrow statutes. The form must be signed under penalty of perjury by an authorized representative of the manufacturer, affirming that all statements and attachments are accurate and complete.
