This form is used by Nonparticipating Manufacturers (NPMs) and Importers to officially designate a registered agent in the state of Connecticut. This form ensures that the registered agent is authorized to receive legal documents and service of process on behalf of the NPM or Importer. The NPM or Importer must provide advance notice to the Attorney General of changes in the agent or address, and the form must be signed and notarized.
