Description
CC-DR-006 - PETITION TO MODIFY CHILD SUPPORT (Family Law Art., Title 12). This form is used in the Circuit Court of Maryland by a parent, guardian, or other legal party to request a change in a previously issued child support order. Filed under Title 12 of the Maryland Family Law Article, this petition is appropriate when there has been a significant change in circumstances since the last child support order was issued. The petitioner must provide basic information about the case, including the names of the parties, the child(ren) involved, and details of the original support order, such as the amount and frequency of payments. The form allows the petitioner to explain changes that justify modifying the support amount. These changes can include increased or decreased expenses for the child(ren), changes in either parent’s income, or a child’s change in eligibility (such as reaching adulthood, marrying, becoming emancipated, or passing away). The petitioner may request the court to increase or decrease the support amount, change how payments are made (e.g., through wage garnishment or direct payment), require the provision of health insurance for the child(ren), or grant other appropriate relief. A financial statement must also be attached, depending on the combined gross income of the parents. www.FormsWorkflow.com
