JD-1711 - ORDER FOR TEMPORARY PHYSICAL CUSTODY (SECURE/NON-SECURE). This form is used in the Wisconsin Circuit Court to document the court’s decision regarding temporary physical custody of a child or juvenile, either in a secure or non-secure setting. It is applied when a hearing is held to determine if there is probable cause to believe the child or juvenile is at risk of harm to themselves or others, or if their parent, guardian, or custodian is unable or unwilling to provide adequate care or supervision. The court assesses whether the child is under jurisdiction for delinquency, juvenile in need of protection or services (JIPS), or child in need of protection or services (CHIPS). The form also addresses whether reasonable efforts were made to prevent removal or to safely return the child home and considers the child’s placement with siblings, if applicable. If placed out of the home, the court may approve a recommended placement, including qualified residential treatment programs, and impose conditions such as electronic monitoring for non-secure placements.
