CC-DR-054 - REQUEST FOR ORDER OF DEFAULT (Md. Rule 2-613). This form is used in the Circuit Court of Maryland when one party in a case—typically the plaintiff—asks the court to enter a default judgment against the opposing party, usually the defendant, for failing to respond to a petition, complaint, or motion by the required deadline. The form requires the requesting party to provide the name and last known address of the non-responsive party, proof that proper service was completed, and the date on which service and the filing of the affidavit of service occurred. It also asks for a statement confirming whether or not the opposing party is in active military service, in compliance with the Servicemembers Civil Relief Act (SCRA).
