WC77 - NOTICE AND PROPOSAL AND APPLICATION FOR A DIVISION INDEPENDENT MEDICAL EXAMINATION. This form, issued by the Colorado Department of Labor and Employment’s Division of Workers’ Compensation, is used to request and initiate the process for a Division Independent Medical Examination (DIME).
