This form issued by the Superior Court of California, County of Los Angeles is used in Small Claims cases to inform parties of their right to present evidence and participate in hearings remotely. The court strongly encourages remote appearances via LACourtConnect, which allows parties to attend hearings through audio or video at no cost.
