REAL ESTATE TRANSACTION FORM. This form from the Virginia State Corporation Commission’s Public Service Taxation Division is used by companies required to file an annual property report to disclose all real estate transactions involving their property. These transactions may include purchases, sales, conveyances, exchanges, dedications, donations, or leases where the company owns the improvements. The form must be submitted at least every six months, and no later than January 31st of the following calendar year. It requests detailed information such as the nature of the transaction, involved parties (grantee/grantor), description of the facility, property size, local map number, consideration amount, deed dates, and any improvements on the property. Companies must also attach copies of the deed and plat when submitting the completed form.
